C-5 Referral Partners
Join the collective: An ecosystem of high-performance specialists
A triage engine is only as effective as the support available behind it. We bring together a network of professional partners who offer specialised support when a risk or challenge is identified. Employees can then choose the support pathway that works best for them.
By joining our ecosystem, you become a critical control measure in an Australian-first psychosocial risk framework. Together, we provide a complete safety net that protects both the employee and the organisation.
Our core support categories
Our triage specialists help clarify the nature of an employee’s concern and outline available support options. This structured approach means that when a referral reaches a Collective partner, there is greater context and clarity about the type of support the individual needs.

Mental & Physical Health
We connect individuals with psychologists, counselors, and lifestyle wellbeing specialists to address anxiety, burnout, and physical health goals that impede their ability to show up and perform.

Work
Addressing the systemic and interpersonal friction within the workplace. Referrals in this category involve specialists in conflict resolution, career coaching, role clarity, and industrial relations to ensure the professional environment remains productive and safe.

Relationships
Personal challenges inevitably bleed into professional performance. We partner with family therapists, relationship counsellors, and communication specialists to help individuals navigate the domestic complexities that impact their mental load at work.

Financial
Financial stress is a primary driver of workplace distraction and absenteeism. Our network includes financial counsellors, financial planners and debt advisors, who provide the practical tools required to regain stability and reduce the weight of economic pressure.

Partner frequently asked questions
How does the referral process work?
Our triage specialists conduct the initial assessment. If a friction point is identified that requires expert intervention (e.g clinical psychology, or financial advice), we match the individual to the most appropriate member of the Collective and provide you with an introduction to the individual.
How do you ensure the quality of the network?
The C-5 network is an invitation-only or application-vetted network. We look for specialists with a proven track record in quality of service and outcome driven performances.
What kind of data feedback do you require?
To help employers manage psychosocial risk, we track high-level, de-identified trends (e.g 30% of referrals this month related to financial stress). While we never compromise individual patient/client confidentiality, we do require partners to provide de-identified outcome data so we can measure the effectiveness of the triage engine.
Can I remain an independent practitioner?
Absolutely. Joining the network does not make you an employee of C-5 Wellbeing. You remain an independent specialist. We act as the triage and referral engine that connects your expertise with the organisations and individuals who need it most.
